What to bring to your appointment

To make your visit with us go smoothly, please bring the following items to your appointments:

• Your insurance card
• Photo ID
• List of all medications
• Co-pay or payment if uninsured

Your Appointment

Please arrive 15 minutes prior to your appointment to register. At that time, you will need to sign a consent to treat form and verify your insurance information. If you have a co-pay or deductible, we will collect it before your appointment.

If you are unable to keep your appointment, please let us know at the earliest possible time so that we may offer your appointment spot to another patient. You will be charged a $25 no show fee if appointments are not canceled ahead of time.
